A Bridge to Better
Today there are over 2,000 adults and young people with learning disabilities and autism who are trapped in a diminished world inside mental health hospitals and many more on the brink of crisis at risk of admission or readmission at any time.
Care Bridge Specialists offers a ‘partnership model’ and dedictates our service to working together with Local Authorities and Clinicial Commissioning Groups to provide vulnerable people with wraparound teams that can support a complex transition home and a more meaningful life with freedom, choice, dignity and control in the community.
Our Practitioners are highly skilled experts with an average of 6 years experience in the field of learning disabilities, autism and mental health. A Care Bridge Specialist has certified qualifications in Mandatory Training, Basic Life Support, Safeguarding Adults and Children Level 3, Learning Disabilities and Makaton, Autism Awareness, Epilepsy and Buccal Midazolam, De-escalation, Breakaways and Physical Interventions and Positive Behaviour Management and Support. We are immensely proud of them and are inspired daily by their calling to be brave, caring and collaborative in order to deliver a heart-led service that enables every individual that we care for to flourish and thrive.
Our Practitioners greatly benefit from the mentorship of our own in-house Intensive Support Team (IST). This team consists of two Board Certified Behaviour Analysts, a Registered Mental Health Nurse, A BACP qualified Counsellor and a dual qualified Registered General Nurse and Learning Disability Nurse. With over 60 years of collective experience between them our IST is well placed to have clinical oversight of the many proven tools, processes, interventions and strategies around ‘Positive Behaviour Support’ and ‘Positive Psychology Approaches’ that our Practitioners use to change lives for the better.

Our Bridging Support Model is designed to assist and support Commissioners, Case Managers, Social Workers and Multi-Disciplinary Teams enabling them to accelerate hospital discharges for people on ‘Dynamic Support Registers’ knowing that there are skilled and expert teams of Care Bridge Specialists who can build relationships, make reasonable adjustments, facilitate capable environments and address the diminished sense of self-worth, isolation and anxiety that might otherwise be a barrier to a successful transition and community living.
The work we do both in and out of the hospital setting to support, teach life skills and encourage reintegration into the community also greatly reduces the likelihood of a future placement breakdown and the revolving door of hospital readmissions.
Our successes are testimony that when we work collaboratively and share accountability for making the world a more predictable, accessible and safe place, we help those that we support to find their place in it.
