Our Approach

The skilled and flexible Practitioner teams that Care Bridge Specialists creates support options in a variety of different scenarios including:

 

Preventing crisis at home

Providing support to people either in their own home, the family home or existing supported or  residential accommodation can prevent crises from escalating into hospital admissions.

 

Preventing further crisis and escalation in hospital 

Institutional settings such as assessment and treatment units still fail to provide the support that people often need. Sadly, this can and does lead to people moving deeper into institutional systems – often into secure environments which can make it even more difficult for people to be discharged back into their communities.

 

Through our expertise and approach:
  • Our in-house Intensive Support Team (IST) and the pedigree and experience of our Specialist Practitioners means that Commissioners have the confidence to accelerate hospital discharges. Plans are able to be made in the knowledge that there is a Care Bridge Team getting to know each person within the hospital setting and building a picture of their past, present and what they want from their future good life. Every single person is different and, in this scenario, having a stable team around someone gives us the opportunity to look at all the intricacies meaning that we can support the development of a truly individualised service design.
  • We share a seat at the table with the Multi-Disciplinary Team, Families, Housing Providers and Advocates. We attend every weekly meeting to share our handovers and daily insights and to help those that we support to make their voice heard. From our initial introduction we get to know and work alongside Ward Managers, Social Workers, Occupational Therapists, Clinical Psychologists and Speech and Language Therapists so that together we can pull in the right people at the right time to make things happen.
  • We give the individual hope that things are moving forward, and that life is about to get better outside of a hospital environment, which contains and restricts their liberties and which is systems based rather than person centred. We are intentional about co-ordinating the holistic support arrangements that people need to leave hospital successfully.
  • We enable the long-term care provider to take a staged approach, recruit a person-centred team based on the information that we are able to share with them and to be upskilled by sharing the benefit of our experience and best practise. In future we stay around in the background with a team that know each person well, ready to support at short notice if there are any issues, providing stability and consistency as people establish their new lives in the community.
